Use this form to request approval to enroll in a Level 2 class or higher

CLASS APPROVALREQUEST FORM

Approvals are required for Level 2 Classes or higher.  Students should not enroll in a Level 2 or higher class until approval is given.  This form is only intended for students requesting to enroll in a higher level class.
